There are three eligibility requirements for Senior Partners Care: First, you must be on Medicare Part A and Part B. Second, you must meet income and asset guidelines to qualify. These amounts change annually and are based on 200 percent of the federal poverty guidelines. The 2008 guidelines are as follows: your monthly income cannot be more than $1,733 if you are single; $2,333 for a couple. Your assets for either single or couples cannot be more than $41,700. The asset limitation does not include your homestead property and one vehicle.
Third, you cannot be enrolled in any of the Medicare Advantage plans, or receive benefits from either Medical Assistance or Qualified Medicare Beneficiary (QMB). Call the Minnesota Senior Federation-Metro Region Health Plan Information Center at (651) 645-0261 ext. 145, weekdays from 10:00 to 12:00 or 1:00 to 3:00. In greater Minnesota, call your Minnesota Senior Federation regional office. You can ask any questions and request an application. You must complete the application and provide adequate financial information for proof of eligibility before you are accepted into the program. Virtually all Twin Cities hospitals and many hospitals and clinics in greater Minnesota participate in the program. Are there any fees for SPC? No mandatory fees or premiums are required, but you will be asked to donate an annual administrative fee of just $28. If you are unable to pay the fee due to hardship, the fee will be waived upon your request. You will also be encouraged to join the Minnesota Senior Federation. Membership fees vary by region in Minnesota, so ask about this when enrolling in SPC. Although Federation membership is not mandatory, it entitles you to ancillary SPC programs, including discounts from participating providers of chiropractic, dental, optometric, podiatric and audiology care, and durable medical equipment.
What must I do to stay on SPC once I'm enrolled? Once per year, you will need to complete an annual update application and submit financial information verifying you are still eligible for the program in order to continue to participate. |
