Long-Term Care Consultation, Elderly Waiver and Alternative CareIn Minnesota, three programs are available to help elders to continue living at home while offering their caregivers needed support. These programs are administered by the county in which you live. For more information, call your county social services department. Long-Term Care Consultation An evaluation of the elderly person's living situation and his/her needs. The evaluation is done by a team of a social worker and a public health nurse who will make recommendations about services needed. Recommendations may range from home care services to supportive living arrangements to nursing home care. The elderly person's preferences, wishes and informal supports are crucial in making any recommendations. The consultation is provided at no cost regardless of the elderly person's financial situation. The screening is also the first step in accessing the Elderly Waiver and Alternative Care programs for eligible persons. This service is available to people of all income levels. Elderly Waiver A home care program for persons who are over the age of 65, eligible for Medical Assistance and at risk of needing nursing home placement. The goal of this program is to help these individuals remain in their home. Factors that are considered are the availability of appropriate home care services and the cost effectiveness of these services. Some of the services provided include homemaker, home health aide, respite care, adult day service and case management. Alternative Care A companion program to the Elderly Waiver and the eligibility guidelines are the same except individuals do not have to be eligible for Medical Assistance. Both income and assets are considered when determining eligibility and there may be a charge for these services. |
