Image HOW TO ORDER:  To place an order, fill out the Member and Account Information Form. This includes a brief health history and a request to provide a listing of other medications you are taking. Please ensure that all entries are filled. You must include your Senior Federation membership number on this form to have your order processed.

Remember, to use this program you must be a current dues paying member of the Minnesota Senior Federation.

Your physician has only to provide a standard prescription for you. Mail the completed form along with copies of your prescriptions to the address provided on the form. A new limitation as of August 18, 2006 is that ECP is unable to accept faxed prescriptions unless they are faxed directly from your doctor's office directly to ECP.

HOW TO PAY FOR YOUR ORDER:  Extended Care Pharmacy accepts payment by VISA or MasterCard. The payer's credit card statement will be in U.S. dollars. Extended Care Pharmacy also accepts online checking, as well as international money orders (if you provide authorization and needed information). Please provide credit card or check information on the Member And Account Information Form. Payment is required before the order is shipped.

MAXIMUM ORDER QUANTITY:  A maximum 3-month supply of any given medication can be ordered. Items can only be shipped in the manufacturer's original, sealed container. Order quantities must be in a multiple of the manufacturer's package size. For example, if a medication is packaged 100 tablets to a bottle, 30 tablets cannot be ordered. You would have to order 100. Where these two rules conflict, as an example, a 3-month supply is 90 tablets and the bottle size is 100, the rule about shipping in the manufacturer's sealed container takes precedent. That means you would have to order 100.

CONFIRMATION OF ORDER:  Be sure to include your phone number or email address on your order.  Once your order is received, a pharmacist will personally review it and you will receive telephone confirmation on your order. If you have not received order confirmation in 72 hours (excluding weekends), please assume that your order has not been received. Please make sure that the doctor and patient's telephone numbers or email addresses are on the order. This will facilitate follow-up questions and order confirmation. Extended Care Pharmacy cannot provide order confirmation unless the patient's phone number or email address is provided.
