One copy of Health Care Choices for Minnesota Seniors is free to Minnesota Senior Federation Members.  The cost to non-members is $8.00. 

To request a copy, please call the MnSF at 651-783-5045 (toll free 1-866-783-5045) from 10:00 a.m. - noon and 1:00 - 3:00 p.m. Monday through Friday.  Non-members may pay by credit card over the phone, and a copy will be mailed to you.
