Name of the employer
Brekadown of hours/months/years worked under plan or with the same employer
All correspondence received or sent regarding the plan
Name of persons spoken to regarding the plan
Former employer's address/location
Type of business (i.e. computer parts manufacturer, etc.)
Any relevant information about the company’s history including moves, closings, shutdowns, bankruptcies, mergers, divisions or buyouts, plus relevant dates, parties or partnerships involved, if available.
Any employment related documents.
Known names and/or city of residence of any of the Officers (CEO, president, etc) or anyone on the Board of Directors of either the selling company or the purchasing company.
Union affiliation, even if it was not a collectively bargained plan.
Information provided by family members, friends, and/or former co-workers.
Any other information that would help identify former or current owners of the company or former or current administers of the pension plan.
